After study, I believe this program is from the December 1, 1968 football game that featured the Miami Dolphins at the New York Jets. It is undated but lists Joe Namath's age at 25 which would place the program just before Namath's miracle at Super Bowl III in 1969 vs. the Baltimore Colts! The program is only four pages long.
